The decidua refers to the specialized layer of the endometrium (the inner lining of the uterus) that forms during pregnancy. It serves as the crucial foundation for the placental bed, playing a vital role in supporting the developing embryo.

The formation of the decidua is a complex process known as the decidual reaction.

What is Decidua?

The decidua is more than just a modified uterine lining; it's a dynamic tissue specifically adapted for pregnancy. It is the site where the embryo implants and where the placenta eventually develops, forming a critical interface between the mother and the fetus.

Key characteristics and functions include:

  • Specialized Layer: It's a unique and temporary layer of the uterine lining, distinct from the non-pregnant endometrium.
  • Placental Base: Provides the foundational support and attachment site for the developing placenta.
  • Maternal-Fetal Interface: Facilitates the exchange of nutrients, oxygen, and waste products between the mother and the fetus, while also offering immune protection.

Understanding the Decidual Reaction

The decidual reaction is the process by which the endometrial stromal cells undergo significant changes to become decidual cells. This transformation is primarily triggered and sustained by the hormone progesterone.

The process involves:

  1. Hormonal Induction: Initially, progesterone produced by the corpus luteum in the ovary stimulates the endometrial stromal cells. As pregnancy progresses, the placenta itself takes over the production of progesterone.
  2. Cellular Modifications: The endometrial stromal cells enlarge, become polyhedral (multi-sided), and accumulate glycogen and lipids, transforming into specialized decidual cells.
  3. Vascular Alterations: Accompanying these cellular changes are significant modifications to the blood vessels within the endometrium, ensuring an adequate blood supply for the growing pregnancy.

  • Protection: Acts as a barrier against potential infections and prevents excessive invasion by the trophoblast (cells that form the outer layer of the blastocyst).
